To receive proper credit please ensure that the information on file with Dun & Bradstreet is accurate and up to date. Credit Agreement Sign and fax to 408-588-1236 to obtain credit terms including COD company check. Self-Insurance Binder - If your company has an insurance policy and does not wish to have the package insured by the carrier, please submit this form together with a copy of your insurance binder. CA Resale Card - Please fax back a copy of this with your resale license if you are a California Resller. Customers out of state do not pay California sales tax. Reseller / Tax Exempt customers must provide a reseller form at the time the order is placed. No credit is to be given once the order is shipped. This list has a privacy policy.
